Your Right to Access Your Health Care Records

You have the right to review and receive copies of your health care and mental health records, including electronic records, as permitted by law.

How to Request Records

Reasonable fees may apply as allowed by law.

 

Requesting Changes to Your Records

If you believe information in your record is inaccurate or incomplete, you may request an amendment in writing. We will review your request and respond in accordance with applicable law.

 

Confidential Communications

You may request that we communicate with you in a specific way or at a specific location (for example, by email instead of phone). We will make reasonable efforts to honor your request.

 

Restrictions on Use or Disclosure

You may request limitations on how your health information is used or shared. While we consider all requests, we are not required to agree to every restriction.

Identity Verification & Attestation

To protect your privacy and prevent unauthorized access to health or mental health records, Unnecessary Hypotheticals Therapy PLLC requires identity verification prior to releasing records. By submitting this form, you confirm that you have uploaded:

  • A government-issued photo ID (driver’s license, state ID, or passport), and

  • A selfie clearly showing you holding the same ID

 

Uploaded identification is used solely for identity verification and is stored securely in accordance with HIPAA and Texas privacy laws. You may redact ID numbers other than your name, photograph, and date of birth.

 

By signing and submitting this form, you attest that the information provided is accurate and that you are authorized to request these records.

 

Important: Requests submitted without the required identification and selfie will not be processed and may be denied until proper verification is provided.
